Phil passed on May 10, 2004 at the age of 60, per U.S. Social Security Death Index.  He was living in Topeka, Kansas.  His cause of death is unknown.

I cannot find article on Phil's death, however, I found a small article on the passing of his wife Sharon, which does include some info.
